Hemorrhoids are increased in volume or expanded vascular formations, which are located in three or four “pads” in the distal part (anal canal) of rectum.
Hemorrhoids is the most common disease of the right colon, are equally common in men and women.
The appearance of hemorrhoids in most people is associated with heavy physical work, a prolonged stay in an upright position, pregnancy and childbirth, chronic constipation. The incidence of hemorrhoids increases with age, as reducing the bond strength of the mucous membrane of the anal canal, with its walls and therefore the hemorrhoidal prolapse appears.
Clinically hemorrhoids can have chronic and acute form. It should be borne in mind that these conditions are not isolated from each other, but the phases of the same process.
For chronic hemorrhoidal disease, the most characteristic manifestation is a bleeding, usually at the end of act of defecation and appearance of prolapsed hemorrhoid out of the anus. In the later stages of the disease, these symptoms can join itching and a secretion of mucus, which is due to the constant hemorrhoid prolapse.
Acute hemorrhoids is characterized by the triad of symptoms – a pain, an increasing of hemorrhoidal nodes, which is based on their thrombosis, and a concomitant inflammatory changes in the surrounding tissues.
If the hemorrhoids is not treated in time, the passage of disease into an advanced stage with constant trauma of nodes during a defecation will lead not only to permanent exacerbations, but also to the development of complications such as chronic anaemia and inflammatory changes in the tissues surrounding the rectum. Thrombosis of hemorrhoids is also unsafe due to migration of blood clots that is risky for the outcome of the disease.
Hemorrhoid treatment without surgery
Hemorrhoid treatment is carried out at any stage, and differs only in applied in the course of treatment methods that are required to be selected, prescribed and controlled by the doctor proctologist.
To treat an internal, external and combined hemorrhoids at the initial stage of its development (stage 1) in most cases is sufficient to have correctly administered conservative therapy. In case of bleeding internal hemorrhoids of stage 1 it is often indicated an infrared laser coagulation of the nodes. The procedure is performed on an outpatient basis during 10-20 minutes, after which the patient keeps calm during one day at home.
For internal hemorrhoids stage 2, depending on structure and size of hemorrhoids an infrared laser coagulation or ligation of nodes with latex rings is indicated.
The procedure is performed in an outpatient basis during 10-15 minutes (ligation of a single node), after which the patient keeps calm during one day at home and in a period of 4-7 days the rejection of the ligated lodes is completed.
In case of internal hemorrhoids of stage 3 with prolapsed hemorrhoid, it is indicated the ligation with latex rings or surgery by the method of Longo, or transanal hemorrhoidal dearterialization (THD) using the ultrasonic sensor or the surgical “gold standard” – surgery of hemorrhoidectomy.
Internal, external and combined hemorrhoids of stage 3-4, require the surgical hemorrhoidectomy.
